Regular, one-to-one support is being provided by a team of Community Connectors as part of specialist mental health services. The programme is designed to help local people manage their diagnosed serious mental illness (SMI) along with their physical and mental health and their wellbeing.
How it works
Community Connectors help people identify what’s important to them and what’s happening in their lives. They support people in one-to-one sessions to connect with services and activities that will aid recovery and help build on strengths, abilities and interests. Services many include: benefits and housing advice, social groups, relaxation, creativity, physical and healthy lifestyles activities.

This approach is at the heart of the work we do at the Bromley by Bow Centre. Our programme aims to achieve greater positive impact on the health and lives of people, groups and communities living with mental ill-health.”
Community Connectors’ work is based on the Centre’s well-known social prescribing approach. They form part of a multi-disciplinary team funded by East London NHS Foundation Trust, working closely with the health, social care and voluntary sectors.
How to get a referral for support
The programme operates alongside eleven health centres in Tower Hamlets primary care networks 5 and 6. In network 6 are Health Centres at Bromley by Bow, XX Place, St Andrews, St Paul’s Way and Wellington Way Network 5 covers Harley Grove, Grove Road, St Stevens ,The Tredegar Practice and Ruston Street Clinic.
Sessions take place at the Bromley by Bow Centre and throughout the local area via a referral from one of the GP practices above or the Community Mental Health Team.
