Social Prescribing Link Worker

The Bromley by Bow Centre is a vibrant community centre in the heart of Tower Hamlets. The Centre is a charity with a national and international reputation for its innovative work in inspiring and empowering people from the local community, using a unique model of integration. It supports over 2,000 people each month.

We are seeking a Social Prescribing Link Worker who will assist the Bromley by Bow Centre and Bromley by Bow Health Partnership to meet the growing needs of our community caused by the current pandemic.

The role will focus on urgent referrals into the Social Prescribing service and proactively contacting people at additional risk of social and economic vulnerability as a result of Covid-19. The role will provide immediate and holistic support over the phone, assessing individuals’ needs and connecting them to local initiatives and services. You will also help shape the Bromley by Bow Social Prescribing service so that it reflects the changing environment and helps reduce health inequalities locally.
